HOUSING TRANSITION NAVIGATION SERVICES
Who Qualifies for this Program
Adults and families who live in LA County receive Medi-cal and work with one of the major insurance plans in the area, LA Care, Healthnet, Anthem (Blue Cross), Molina, Kaiser, Blue Shield.
Meet the HUD definition of homelessness or be at high risk of becoming homeless
Has at least one chronic condition mental or physical, documented by ECM or medical provider
HOUSING NAVIGATION
Housing programs can assist with:
Finding housing that matches your income and family circumstances
Finding Community resources that can help with moving in cost (depending on your insurance)
Finding Emergency Shelters in your area
Assist with applying for available voucher programs as they become available
Assist with applying for housing applications
Advocate with your landlord on your behalf
Provide resources to assist with maintaining current housing or starting a new one.
Counseling on money management and budgeting
Direct members to other services like job opportunities, legal services, food pantry, and other community services
