Activity Service Coordinator

Under the direction of the Senior Services Manager the Activity Service Coordinator supports the operations and management of the Central Neighborhood Health Foundation and provides administrative support to the Community Benefits. 

EDUCATION AND/OR EXPERIENCE: 

Community Health Worker with 1 to 2 years’ experience in clinical or community resource settings; Care coordination and/or case management experience is desirable.  Evidence of essential communication, education, and counseling skills.  Proficiency in communication technologies (email, cell phone, etc.).  Highly organized with ability to keep accurate notes and records.  Experience with health IT systems and reports is desirable local knowledge about and connections to community health care and social welfare resources is desirable.  Ability to speak a relevant second language is desirable. Minimum High School Diploma or equivalent.

Outreach Coordinator

Upholds and supports the philosophy, objectives and policies of the office under the direction of the office manager and physician(s)

ESSENTIAL DUTIES AND RESPONSIBILITIES 

Include but are not limited to the following functions. Must be certified with Covered California Bilingual Spanish speaking preferred.  Pleasant and sensitive to the needs of the patients.  Enroll patients in Covered California marketplace.  Enroll patients for Medi-Cal.  Enroll patients for My Health LA (MHLA) & County programs.  Assist patients to submit health options and medical choice forms.  Call health plan for Primary Care Physician (PCP) changes.  Educate and advise patients on available insurance and plans.  Monitor and control Transportation spreadsheet and schedule patients for transportation to and from doctor appointments and to the pharmacy.  Provide tokens for patients when requested. Provide assistance to patients for transportation through their insurance.  Conduct outreach to increase patients roster.  Submit weekly reports regarding enrollments completed.  Assist patients with available resources regarding housing and/or employment in an effort to meet their specific needs.

Chronic Care Management Coordinator

ESSENTIAL DUTIES AND RESPONSIBILITIES

 Chronic Care Management (CCM) model involves services provided by a care team including a care coordinator, physician, Nurse Practitioner (NP), Physician Assistant (PA), Certified Nurse-Midwife (CNM), or Clinical Nurse Specialist (CNS) - and their clinical staff. The teamprovides support to patients with chronic conditions, coordinates the patients’ physical and behavioral health care services and connects them to community services and housing, as needed. The care coordinator may check in weekly, biweekly or monthly over the phone, and/or in person with patients. The services involved in CCM are comprehensive, involving maintaining a comprehensive care management, completing electronic care plan, performing care coordination, transitions of care along with other care management services, recording structured patient health information, and timely communication of patient health information, providing health promotion and services to help members with community and social services such as housing, transportation, and food. 

QUALIFICATIONS 

Minimum of 3 years’ experience as a Care/Case Manager or Coordinator either in the Homecare, Inpatient, Physician Practice or in home case management setting.  Experience in clinical telephone triage.  Experience working with Electronic Health Record (EHR)

EDUCATION and/or EXPERIENCE 

Minimum High School Diploma and 3 years experience working in Homecare, Inpatient or Physician Practice.
