Diabetes Comprehensive Care Program

Our Diabetes Comprehensive Care Program is specifically designed for individual needs. 

CNHF offers a long-term, personalized, and team-based approach to diabetes care specific to individual needs to help you manage the condition.

Personal Health Coach: The uniqueness of our program is that once the patient provides consent to be enrolled in the program, the patient is assigned to a Care Coordinator and Diabetes Peer Educator. Together, they will create a flexible plan designed specifically for the patient, to address their physical health, mental health, and social service needs. All personal training packages will include:

  • Lifestyle evaluation
  • Progress and final evaluations
  • Realistic goal-setting
  • Supervised fitness sessions
  • Thorough fitness assessment

Care Coordinator: In addition to face to face consultation, the Care Coordinator will provide at least 20 minutes of non-face-to-face or in-person care management service per calendar month. The Care Coordinator will use a predetermined database of resources to provide:

  • Foodbank referrals
  • Clothing assistance referrals
  • Housing referrals
  • Utility bill assistance program referrals
  • Substance abuse counseling, behavioral health, and mental health referrals 
  • Transportation resources
  • Breast cancer screening referrals 
  • Nutrition referrals 

Currently, CNHF is actively engaged with the LA County Department of Public Social Services (DPSS) and obtained a CalFresh Application Assist number to help our low-income patients apply for CalFresh benefits at each CNHF Clinic location. 

Comprehensive Diabetes Classes: We offer a variety of ongoing diabetes education classes and workshops in English and Spanish to learn healthy lifestyle habits such as cooking and fitness classes and nutrition counseling. For all other languages, we can provide an interpreter, as needed.
